Creating a form in Dreamweaver can be fairly simple but actually making it work needs a CGI script to be put in place. If you are not familiar with this process, you might want to try a readymade from one of these sites:

If you plan to have a go with a CGI (common gateway interface) script yourself, you might find forms to go useful to help you create a script or you could try nethermail - an advanced form mailer that can be used for any type of contact form.

You will also need an ftp program that will allow you to upload files as ASCII text and also allow you to set file permissions. Here are a few suggestions - for Mac I like Transmit and Fetch, for PC try filezilla or WS_FTP
